We make transmural care work in practice by standardizing integration between hospitals and remote monitoring providers, so patient data reaches the right caregiver, faster and securely.

In 2021, telemonitoring integration was still fragmented and inconsistent. Transmural Platform (TMP), originally called Telemonitoring Prescription, introduced a standardized request and return data flow between hospital EHR systems and external providers, simplifying connections and improving continuity of care.
